$50 charged to the payment method on file.Late Arrivals: If you arrive late, your session may be shortened to accommodate subsequent patients. You remain responsible for the full service fee.5. Payments, Packages, and RefundsPayment Due: Payment is required in full at the time of service or product purchase.Non-Refundable Policy: All medical procedures, cosmetic injectables, laser treatments, opened skincare products, and custom BHRT prescriptions are strictly non-refundable.Package Expiration: Prepaid treatment packages or gift certificates must be fully utilized within 6 months of the purchase date. Unused balances automatically expire and are forfeited after this 6-month timeframe.Insurance Disclaimer: Most aesthetic services and BHRT programs are elective and not covered by commercial health insurance or Medicare. You are solely responsible for all out-of-pocket costs.6. Membership Subscription ClauseRecurring Billing: By enrolling in a Genesis Aesthetics membership tier, you authorize monthly recurring charges to your designated credit card or payment method on file. Charges occur automatically on the same calendar day each month.Commitment Period: Memberships are subject to any minimum commitment periods specified at the time of enrollment.Cancellation Policy: To cancel a monthly subscription, you must submit a written cancellation request via email or through our patient portal at least thirty (30) days prior to your next scheduled billing date.Forfeiture of Unused Benefits: Upon successful termination of your membership, any remaining, unused monthly credits or promotional benefits must be redeemed within thirty (30) days or they will be permanently forfeited.7. Communications (SMS and Email)Consent: By providing your phone number and email, you opt in to receive operational communications (appointment reminders, pre / post-care instructions) and promotional updates from Genesis Aesthetics.Opt-Out: You can opt out of marketing messages at any time by replying "STOP" to text messages or clicking "Unsubscribe" in emails. Standard messaging rates apply.8. Limitation of LiabilityInherent Risks: Medical spa treatments and hormone therapies carry inherent clinical risks (including but not limited to bruising, swelling, allergic reactions, changes in mood, laboratory fluctuations, or skin discoloration).Assumption of Risk: By signing your clinical informed consent forms prior to treatment, you voluntarily assume these risks.Maximum Liability: To the maximum extent permitted by law, the Clinic’s total liability for any claim arising out of your use of our services or website shall not exceed the total amount paid by you to Genesis Aesthetics in the 12 months preceding the claim.9. Privacy & HIPAAYour personal health information (PHI) is protected under federal and state privacy regulations. Please review our separate Notice of Privacy Practices (HIPAA) to understand how we secure, utilize, and disclose your medical records.10. Governing Law & VenueThese Terms are governed by and construed in accordance with the laws of the State of Wisconsin, without regard to its conflict of law principles.
